The Department of Human Resources is responsible for attracting, motivating and retaining the most qualified employees to ensure the effective operations of the Park District. The work of this department encompasses a coordinated effort with each department and region to attract and retain qualified individuals in order to enhance the success of the organization. The department specifically manages benefits, compensation, job classification, compliance, rules, candidate screening, policies and procedures, and collective bargaining agreements along with the related labor relations functions.

1095-C Form

The Chicago Park District is no longer required to automatically furnish employees with their 1095-C form. If you would like a copy of your Form 1095-C, go to paperlessemployee.com\cpd to download it. If you are unable to download your 1095-C form electronically and would like to request a copy, please email benefits@chicagoparkdistrict or call (312) 742-0923. 

    Form 1095-C is an IRS document issued by employers. It shows the health insurance offered to an employee in a calendar year and the months they were covered. It also includes information about dependents’ coverage.

    No, do not send the form to the IRS when you file your income taxes. However, you will need to report your health insurance coverage to the IRS when you file your taxes. Use the 1095-C form to help answer questions about your coverage.

    Keep a copy with your tax records. The information may be needed if you or a dependent applies for coverage through the Affordable Care Act’s health insurance marketplace. The information on the 1095-C form helps verify if requirements have been met for premium tax credits.
